A Care Manager is a nurse who meets with you or calls you and works with you to find solutions to barriers that may prevent you from following your treatment plan.
A Care Manager works one-on-one with you to help you learn about your medical condition and what you can do to improve it.
If you have a chronic illness or are having trouble managing a health condition, your doctor may have a Care Manager contact you.
