A Hospitalist is a medical doctor who specializes in caring for patients in the hospital. Our Hospitalists team up with Care Managers, who are registered, nurses. Together, they see our patients in the hospital and make sure they get the right care at the right facility.
If you are admitted to an Optum contracted hospital, a Hospitalist will spend time with you and your family going over your medical history, your medications, and your treatment.
When It's Time to Leave the Hospital
When you are ready to leave the hospital to go home or to another care setting like a skilled nursing center, a Care Manager will work with you and your family or caregivers to help make that transition smooth. Your Care Manager may assess the home setting you'll be returning to, ensure that you have any medical equipment or home health services you may need, and more. He or she will also make sure your primary care doctor knows the details of your hospital stay.
